First Health Part D is a Medicare approved national prescription drug plan underwritten and administered by Coventry Health Care plans (Coventry Health and Life Insurance Company, Cambridge Life Insurance Company, and First Health Life & Health Insurance Company) and marketed by distribution partner. The Coventry Health Care plans are solely responsible for all First Health Part D contractual obligations including network management and claims payment. Distribution partner and the Coventry Health Care plans are independent, unaffiliated companies.

Members may enroll in the plan only during certain times of the year. For details, contact First Health Part D customer service at 1.800.588.3322, 8 a,m, to 8 p,m,, local time/seven days a week (TTY/TDD 1.888.788.4010). You must use network pharmacies to access your drug benefits, except under non-routine circumstances when you cannot reasonably use a pharmacy in the network. The First Health Part D network includes retail, long term care, Indian/Tribal/Urban pharmacies and Home Infusion pharmacies. Generally, benefits are only available at the contracted network pharmacies. This document is available in other formats. You must be entitled to Medicare benefits under Part A or enrolled in Part B and continue to pay your Part B premium (if not otherwise paid for by another third party) and reside in the service area. You may only be enrolled in one Part D plan at a time. If you are enrolled in a MA coordinated care plan (HMO or PPO) or an MA PFFS plan that includes Medicare prescription drugs, you may not enroll in a PDP unless you disenroll from the HMO, PPO or MA PFFS plan. If you are enrolled in a PFFS that does not provide Medicare Prescription drug coverage, an MA Medical Savings Account (MSA) plan or an 1876 Cost plan, you may enroll in First Health Part D.
